A 35-year-old male presented to the Emergency Department with a history of pain, redness and swelling in the left leg for the past 5 days. A 2-zone compression test was performed in both lower extremities by the Emergency Room physician. The image and video above were obtained in the left popliteal region at the level of the popliteal crease. The image shows the popliteal vessels with adequate compression applied at the level of the popliteal crease.

Based upon the observation of the image and video, what is the correct diagnosis?

Clot is seen in the popliteal vein.

A 35-year-old Asian female presented to the Emergency Department with a chief complaint of pain and swelling in the left leg. The video clip above was obtained while performing a two-zone compression test at the level of the left popliteal crease to look for evidence of DVT. The compression test was performed as per protocol.

What is your conclusion on observing the video?

Non-compressible popliteal vein - DVT present

Thrombus seen in the popliteal artery

Normal compressible - no DVT present.

A 65-year-old Hispanic male presented to the Emergency Department with pain and swelling in the left leg after a long flight from Australia to Chicago. After a POCUS deep vein thrombosis (DVT) assessment, he was found to have a thrombus in his left popliteal vein. The left popliteal vein is a deep vein. Fortunately, no other clots were discovered on a repeat full DVT assessment study the next day.

What is a key difference between a deep vein and superficial veins?

Deep veins are always accompanied by an artery .

Deep veins do not have valves.

Deep veins are always 4-5cm deeper than superficial veins.
